What do you need for your appointment

Please remember to:

  • Bring your valid Medicare | DVA | Seniors Card
  • Bring a list of any medications you are taking
  • New patients please print and fill in our new patient form – New patient form or;
  • Arrive 10 minutes prior to your appointment to fill out the necessary paperwork.

We’ll try to book you in for an appointment with your preferred doctor and time. If we can’t offer you your first choice we’ll find the next best appointment available. If you’re unable to attend your appointment we ask that you contact us and give as much notice as possible.

Most of our GP’s have undertaken additional specialist training and have been awarded with The Fellowship of the Royal Australian College of General Practitioners (FRACGP). This is a specialist general practice qualification accredited by the Australian Medical Council.

FRACGP denotes safe, specialised, high-quality general practice care.

Fees

Majority of our Standard GP consultations are bulk-billed for patients who hold a Medicare card.

Some consultations may incur a fee including:

  • Pre Employment Medical
  • Superannuation Paperwork
  • Commercial/Heavy Vehicle Licences

Please check with reception regarding the fees.

You can pay any consultation fees or outstanding accounts by:

  • Eftpos
  • Visa
  • Mastercard
  • Cash.

Your rights

If you’re having any concerns in regard to the care provided to you from Moonee Ponds Super Clinic, please contact our Practice Manager or discuss the issues with your GP.

You may prefer to e-mail through your concerns which is more then welcome. Contact our reception staff today for the practice managers e-mail address. However, if you feel the need to discuss this outside of our practice please contact The Medical Registration Board, AMA or Health and Disability Services Complaints Office.
